The contractor file: which papers go in, by role

RN, LPN, CNA and HHA roles have different requirements. Organize each file by role to identify the right evidence and avoid outstanding items that do not apply.

What everyone carries

  • The completed application and the independent contractor agreement signed by both parties — no blank fields.
  • Level 2 background screening (FS 408.809), verifiable against AHCA’s Clearinghouse — and renewed on time.
  • Current CPR.
  • W-9 and the contractor’s tax documentation.

What changes by role

Check current RN and LPN licenses and CNA certification against the official source. For HHAs, retain the training and competency evidence required for their role.

The typical mistake is the one-size template: asking the HHA for the nurse’s paper (or the reverse) fills the file with false "missing" flags and hides the real ones. In a real agency’s physical folder, each role’s file has its own tabs — and that is how it should be watched.
